Supporting You to Live Independently at Home

The Support at Home Program is designed to help older Australians remain independent, safe and connected while continuing to live in the comfort of their own homes.

At Caring Hands Community Support Services, we provide personalised support based on your needs, goals and lifestyle. Our team works with you and your family to make sure your care is practical, respectful and centred around what matters most to you.

Whether you need help with everyday activities, personal care, nursing or other approved supports, we are here to make life at home easier and more comfortable.

What is the Support at Home Program?

Support at Home is the Australian Government’s in-home aged care program. It replaced the Home Care Packages (HCP) Program and Short-Term Restorative Care (STRC) Programme from 1 November 2025.

The program aims to help older people:

  • Stay living safely at home for longer

  • Maintain independence and choice

  • Access the right support for their assessed needs

  • Stay connected with family and their community

  • Maintain their health, wellbeing and quality of life

Support is based on an individual’s assessed needs, goals and circumstances.

Personal Care

Support with personal care and everyday activities while maintaining your dignity, privacy and independence.

Domestic Assistance

Help with household tasks to keep your home clean, comfortable and safe.

Meal Preparation

Support with preparing nutritious meals and maintaining healthy eating routines.

Community & Social Support

Assistance to stay connected with your community, attend activities and maintain meaningful social connections.

Transport & Appointments

Support to attend appointments, access essential services and participate in the community, where approved under your support plan.

Nursing & Clinical Support

Access to appropriate nursing and clinical supports based on your assessed needs and approved services.

Allied Health & Therapy

Support may include appropriate allied health services and therapy supports where these are included in your approved care plan.

Home Maintenance

Assistance with eligible home maintenance tasks that help you continue living safely and comfortably at home.

Support at Home Service Categories

Support at Home services are broadly organised into three categories: Clinical Supports, Independence Services and Everyday Living Services.

Clinical Support

Clinical services are focused on your health and wellbeing and can include services such as:

» Nursing care

» Wound care

» Allied health services

» Therapy and rehabilitation

» Medication-related support

» Other approved clinical services

The Australian Government funds the full cost of clinical supports under the program.

Independence services

These services are designed to help you maintain your independence and continue participating in everyday life.

Examples may include:

» Personal care

» Social support

» Respite

» Transport

» Assistance with daily activities

» Other approved independence supports

Everyday Living services

These supports help with practical tasks around your home and everyday life.

They may include:

» Domestic assistance

» House cleaning

» Meal preparation

» Shopping assistance

» Gardening

» Home maintenance and repairs

» Other approved everyday living supports

How Does Support at Home Work?

1. Start with My Aged Care

Your Support at Home journey begins through My Aged Care and an aged care assessment. The assessment considers your needs, strengths, goals and the support you require to remain living independently at home.

2. Receive Your Assessment & Funding

If you are approved, you will receive information about your funding and approved supports. Support at Home includes ongoing funding classifications as well as short-term pathways for specific needs.

3. Choose Your Provider

You can choose a registered Support at Home provider that suits your needs and preferences.

4. Develop Your Support Plan

Your provider works with you to understand your goals, preferences and daily needs and develop an individualised approach to your care.

5. Receive Your Services

Your approved services are delivered according to your support plan, helping you remain safe, independent and connected at home.


Short-Term Support Pathways

Support at Home also includes short-term pathways for people who require additional, time-limited assistance.

» Assistive Technology & Home Modifications

The Assistive Technology and Home Modifications (AT-HM) Scheme can provide funding for approved equipment, technology and home modifications that help support independence and safety at home.

» Restorative Care Pathway

The Restorative Care Pathway provides time-limited support designed to help people improve their function, regain independence and reduce the need for higher levels of ongoing care. It can provide support for up to 16 weeks.

» End-of-Life Pathway

The End-of-Life Pathway provides additional in-home aged care support for eligible people who are approaching the end of life and wish to remain at home where possible.
